uterine involution
Uterine involution is the process by which the uterus, hugely enlarged by pregnancy, shrinks back down toward its normal size after birth. Imagine an inflated balloon slowly and steadily deflating and tightening over the weeks following delivery.
Immediately after the placenta is delivered, the uterus must clamp down to stop bleeding from the site where the placenta was attached. Oxytocin, released in part by ongoing breastfeeding, makes the uterine muscle contract firmly, which both compresses blood vessels to limit blood loss and helps the organ progressively shrink. Over the following weeks the muscle cells reduce in size and the uterus returns toward its pre-pregnancy dimensions.
This is why breastfeeding in the early postpartum period is often accompanied by cramping (afterpains): suckling triggers oxytocin, which contracts the uterus and aids its recovery while also reducing bleeding.
A clinical caveat: when involution fails to proceed normally, called subinvolution, often due to retained placental tissue or infection, it can cause prolonged or heavy postpartum bleeding, which is a recognized obstetric emergency requiring prompt evaluation.
