milk let-down reflex
The milk let-down reflex is the moment milk actually flows. Milk is made and stored deep in the breast, but it does not come out on its own; the let-down reflex is the body's way of squeezing it forward so the baby can feed, almost like wringing out a sponge from the inside.
It is a neuroendocrine reflex driven by oxytocin. When the baby suckles, sensory nerves in the nipple signal the hypothalamus, which prompts the posterior pituitary to release oxytocin into the blood. Oxytocin then contracts the myoepithelial cells, tiny muscle-like cells wrapped around the milk-filled alveoli, squeezing milk into the ducts and out through the nipple.
Notably, this reflex can be conditioned. The sight, sound or even thought of the baby can trigger let-down before suckling begins, while stress, pain or anxiety can inhibit it by interfering with oxytocin release.
A practical caveat: a poor or delayed let-down is often situational, not a sign of low milk supply. Because emotional state strongly modulates oxytocin, relaxation and comfort can matter as much as physical stimulation for milk to flow well.
