Think of how an ordinary chair lets a tired person slump — and now imagine someone whose muscles cannot hold their own trunk, head, or pelvis upright at all. A postural support system is the network of cushions, backrests, headrests, lateral pads, belts, and trays built into a wheelchair that physically holds such a body in a stable, useful position so the person can function instead of fighting gravity all day.

Each component does a specific job. The seat cushion and a well-positioned pelvic belt anchor the pelvis, which is the base of the whole structure. A contoured backrest and lateral trunk supports stop the upper body from sliding sideways or collapsing forward. A headrest steadies the head for someone with weak neck control; thigh and foot supports position the legs; a tray gives the arms somewhere to rest and work. Crucially, every support is positioned to do one of three things: stabilise (hold a body part still so other parts can move usefully), align (keep the body in good posture), or accommodate (gently support a part that is already fixed in a deformity). The art is supporting enough for safety and function without trapping the person or blocking the movements they do have.

Postural support is where seating turns from a comfort question into a function-and-health question. With the trunk steady, a person can breathe and swallow more safely, lift their head to make eye contact, and use their hands for eating, communicating, or driving a power chair. Without it, the body slides into harmful postures that distort the spine and load the skin. The honest tension here is that support and freedom pull against each other: too little support and the body collapses; too much and you have strapped someone into a position that suppresses the function and movement you were trying to enable. Good seating finds the least restrictive support that achieves the goal.

A child with cerebral palsy who used to slide sideways in his chair is fitted with lateral trunk supports and a contoured back; with his trunk now steady, he can hold his head up and use a communication switch with his hand.

Stabilising the trunk frees the head and hands for function — support enables movement elsewhere.

Support and freedom trade off: too little and the body collapses, too much and you suppress the very function and movement you meant to enable. Aim for the least restrictive support that works.
