hormone assay

A hormone assay is a laboratory measurement that tells you how much of a particular hormone is floating in someone's blood, urine, or saliva. Think of it like reading the fuel gauge of a car: the hormone itself is invisible to the eye, but the assay translates its presence into a number you can act on.

The challenge is that hormones circulate in tiny amounts — often a billionth or trillionth of a gram per milliliter — mixed in with thousands of other molecules. So a good assay must be both sensitive (able to detect very low levels) and specific (able to tell the target hormone apart from look-alikes). Most modern assays achieve this with antibodies that bind only the hormone of interest, or with mass spectrometry that separates molecules by their exact mass.

Interpreting the result is rarely just 'high or low.' Many hormones swing with the time of day, meals, the menstrual cycle, sleep, or stress, so the same blood drawn at 8 a.m. versus midnight can read very differently. Clinicians therefore pair the raw number with the patient's context, a matching reference range, and sometimes a dynamic test that watches the hormone respond rather than reading a single snapshot.

To check thyroid function, a lab typically measures TSH alongside free T4; a high TSH with a low free T4 points to an underactive thyroid.

A two-hormone reading is interpreted as a pair, not in isolation.

Most assays measure the total hormone (free plus protein-bound), but it is usually the free fraction that is biologically active; conditions that change carrier-protein levels, such as pregnancy, can shift the total without the free hormone actually being abnormal.
