Enteroendocrine cells are hormone factories sprinkled throughout the lining of your gut. Rather than being clustered into a single gland, they sit one by one among ordinary digestive cells, each acting as a tiny taste-and-signal post that senses what you have eaten and replies with hormones.

Collectively, these scattered cells make the gut the body’s largest endocrine organ. Different subtypes specialize in different messengers: L-cells make GLP-1 and PYY, K-cells make GIP, I-cells make CCK, S-cells make secretin, G-cells make gastrin, and certain cells make ghrelin or serotonin. Many have a sensing surface facing the gut lumen and a hormone-releasing base facing the bloodstream and nerves, so they translate the chemistry of a meal into endocrine, paracrine, and neural signals.

Because they regulate insulin, appetite, gastric acid, and motility, enteroendocrine cells are central to modern metabolic medicine — the incretin and GLP-1 drug classes essentially mimic or amplify what these cells already do. A caveat: any single cell may release more than one hormone, so the old “one cell, one hormone” picture is an oversimplification.

Though they make up only about 1 percent of gut lining cells, enteroendocrine cells span the largest surface in the body, which is why their combined hormone output is so substantial.
