covalent off-target binding

Some drugs are deliberately designed with a reactive part, a warhead, that forms a permanent chemical bond with their target protein. Covalent off-target binding is what happens when that reactive part bonds, irreversibly, to the wrong proteins instead. Because the bond does not let go, even a small amount of mis-targeting can accumulate and cause lasting trouble.

The danger is sharper than for ordinary off-target binding because covalent attachment is permanent: the modified protein cannot simply release the drug and recover. Such off-target adducts can disrupt protein function, and proteins carrying a foreign chemical tag can be flagged by the immune system, a suspected trigger of idiosyncratic and immune-mediated reactions. The reactive warhead is the source of both the drug's intended action and this liability.

Designing safe covalent drugs is a balancing act: the warhead must be reactive enough to bond its target efficiently but tame enough to ignore everything else. Chemists tune warhead reactivity and shape the molecule so it docks selectively at the target before reacting, and they map covalent off-target binding using mass-spectrometry-based methods that catalog which proteins the drug has tagged.
