Idiosyncratic toxicity is a rare, severe reaction that strikes only a small, unpredictable subset of patients, and that does not follow from the drug's intended mechanism. It is the kind of toxicity that gives drug developers nightmares: a drug can look perfectly safe in thousands of people, then cause sudden liver failure or a catastrophic skin reaction in one patient out of ten thousand.
Because these reactions are not simply dose-driven and depend on a particular person's genetics and immune system, they are extremely hard to predict or reproduce in the lab or in animals. Many are thought to involve reactive metabolites that bind to proteins and trigger an immune response, sometimes linked to specific immune-system gene variants. The delay before onset, often weeks, further obscures the cause.
Idiosyncratic reactions are often invisible until a drug reaches large populations, so they are a major reason for post-marketing withdrawals. Discovery teams cannot screen them out directly, but they reduce the odds by limiting reactive-metabolite formation, lowering the daily dose, and avoiding known structural alerts.
