off-target toxicity
Off-target toxicity is harm that arises when a drug binds to proteins other than the one it was designed for. A drug is meant to be a key cut for a single lock, but if its shape happens to fit other locks too, opening those can cause unwanted and sometimes dangerous effects.
Because the body contains many proteins with similar binding pockets, a molecule rarely binds only its intended target. Off-target binding to an unrelated receptor, enzyme, or ion channel can produce side effects ranging from mild to lethal; hERG block is one notorious example. The harm comes not from the drug's mechanism of action but from interactions it was never supposed to have.
Improving selectivity is a central goal of lead optimization. Teams profile candidates against broad panels of receptors and channels to map their off-target activity early, then redesign the molecule to weaken the unwanted interactions while preserving potency at the real target. A clean off-target profile is a key part of choosing which compound advances.
