Hands are easy to take for granted until they stop cooperating. A wrist that drops and cannot lift, fingers that curl shut and will not open, a thumb that cannot reach the other fingers to pinch — each turns ordinary tasks into struggles. Upper-limb and hand orthoses are braces and splints for the arm, wrist, and hand that hold these parts in useful positions, protect them, or help them work.
They fall into three broad families by what they do. A resting splint simply holds the wrist and hand still in a safe, comfortable position — often used at night, or in a hand at risk of stiffening, to prevent the fingers and wrist from drawing into a tight, useless posture. A functional (static) splint positions the hand so it can do something it otherwise could not: a wrist-hand orthosis that holds a weak wrist straight, for instance, lets the long finger tendons work and turns a floppy hand into a grasping one. A dynamic splint adds a moving element — a spring or elastic — that assists or resists a motion (covered separately).
Hand orthoses appear after stroke, in spinal cord injury, in nerve injuries, in arthritis, and after hand surgery. The art is matching the device to the goal and the time: a joint that needs to rest is splinted still, while a joint that needs to move must not be over-immobilised, or it stiffens. Because the hand is so intricate and its skin so important for feeling and function, fit is delicate, and a splint that is comfortable, worn as intended, and removed for movement and washing is worth far more than an elaborate one left in a drawer.
A woman with a wrist that drops after a nerve injury cannot make a useful grip — her hand collapses backward. A wrist-hand orthosis holds the wrist gently straight; now her finger tendons can pull and she can pick up a cup, even while the nerve slowly recovers.
Resting splints hold a hand safe; functional splints position it to work — match the device to the goal.
A joint that needs to move must not be over-splinted, or it stiffens. Many splints work only if worn on schedule and removed for movement and skin care — an elaborate splint left unused helps no one.
