transthoracic echocardiogram

A transthoracic echocardiogram is the standard, everyday echo: the ultrasound probe is placed on the outside of the chest, and the doctor or sonographer looks at the heart through gaps between the ribs and below the breastbone. The patient simply lies down, gel is applied, and the probe is angled to catch the heart from a few standard windows. It is painless, takes no special preparation, and is done in clinics, wards, and emergency rooms.

From these external windows the operator builds a full picture: chamber sizes, the thickness and motion of the heart walls, how vigorously the left ventricle squeezes, the appearance and function of all four valves, the pericardium, and the great vessels as they leave the heart. Doppler measurements added during the study estimate pressures and flow. For the vast majority of cardiac questions, the transthoracic study is the first and often only imaging test needed.

Its limitation is that sound must cross skin, fat, muscle, ribs, and lung before reaching the heart, so image quality depends on the patient's body and the acoustic window. Structures at the back of the heart — the left atrium, the mitral valve from above, and the atrial septum — are farther from the probe and harder to see clearly. When these areas matter, a transesophageal study brings the probe much closer.

A patient with a new murmur has a transthoracic echo, which shows a thickened, calcified aortic valve and lets the doctor measure how severely it is narrowed.

TTE is the usual first-line test for a heart murmur.
