abnormal synergy patterns
Picture trying to bend just your elbow, but every time you do, your shoulder hikes up and your fist clenches whether you want them to or not — the joints are glued together so the limb moves only as one stiff package. That involuntary 'all-or-nothing' bundling of movements is what clinicians call an abnormal synergy pattern. It is one of the most frustrating barriers between a stroke survivor and a useful arm or leg.
Normally the brain can fire muscles selectively — straighten the elbow while keeping the shoulder still — a skill called fractionation. After a stroke damages the fine cortical control, the limb falls back on cruder, hardwired whole-limb patterns. In the arm there are two classic ones: the flexor synergy pulls the shoulder up and back, bends the elbow, and curls the wrist and fingers; the extensor synergy pushes the shoulder forward, straightens the elbow, and turns the palm down. The leg has its own flexor and extensor versions. The trouble is that real tasks need mixed movements — reaching forward needs a straight elbow with an open hand, a combination the pure synergies cannot produce.
In rehabilitation, escaping these patterns is a key milestone (it corresponds to the breakthrough around Brunnstrom stage 4). Modern therapy does not just wait; it practises the specific 'out-of-synergy' movements the person needs for real tasks, often breaking a goal into the exact joint combinations that the synergy resists. An important nuance: a synergy is not entirely useless — the extensor synergy can sometimes be borrowed to bear weight in standing. The aim is selective control, not simply abolishing all coupled movement.

Therapy targets exactly the missing pieces: elbow extension and finger opening while the shoulder stays down.
Synergies couple joints together; useful tasks need the joints to move independently.
A common misconception is that all synergy is bad and must be suppressed. Some patterns can be harnessed (for example to bear weight), and the real goal is selective, fractionated control rather than blanket elimination.
