task-specific training
There is an old way of doing stroke therapy that, in hindsight, seems oddly indirect: have the person do general strengthening, range-of-motion, and 'preparatory' exercises, in the hope that one day this would add up to being able to, say, drink from a cup. Task-specific training takes the opposite, common-sense view: if the goal is to drink from a cup, then practise drinking from a cup — over and over, with real cups, in real situations — because the brain learns best the very thing it is asked to do.
The idea rests on how the brain relearns. The recovering brain rewires according to what it practises (this is experience-dependent plasticity), and that rewiring is specific — practising a meaningful, whole task drives more useful change than practising abstract movements that resemble it. So task-specific training picks goals that matter to the person, breaks them into achievable wholes, and then drills them with high repetition, gradually increasing difficulty as the person improves. The task is real and purposeful (reaching for a glass, standing up from a chair, stepping over a curb), the practice is plentiful, and the challenge is tuned to be hard but doable.
In rehabilitation task-specific, repetitive practice is one of the better-supported principles of motor recovery, and it is the engine inside several brand-name therapies (such as constraint-induced movement therapy). It also exposes an uncomfortable truth: ordinary therapy often delivers far fewer repetitions than the brain needs, so 'more practice of the right thing' is frequently the most powerful lever available. The honest caveats are that training is specific — practising one task does not automatically transfer to others — and that not every lost function can be trained back; some goals are genuinely out of reach and are met with compensation instead.
Instead of having a stroke survivor squeeze putty and lift weights in the abstract, her therapist sets up her actual goal — picking coins off a table and putting them in a purse — and she repeats it dozens of times a session, with the table arranged to make it just hard enough. Her coin-handling improves markedly, though her separate goal of buttoning shirts needs its own dedicated practice.

A common misconception is that strengthening and 'preparatory' exercises automatically translate into everyday skills — they often do not, because learning transfers poorly. The other honest limit: high repetition helps, but it cannot recover a function the damaged brain can no longer support.
