Lift heavy things for a few weeks and you become strong; jog regularly for a few weeks and you become tireless — but, tellingly, not the other way around. The body does not get generically 'fitter'; it remodels itself in the specific direction you train it. Comparing strength and endurance adaptations shows just how targeted those changes are.

Strength (resistance) training, with heavy loads and few repetitions, first teaches the nervous system to recruit more muscle fibres at once and fire them in better coordination — which is why early strength gains appear before any visible muscle growth. Over weeks the muscle fibres themselves thicken (hypertrophy), especially the fast-twitch ones, and tendons and bone strengthen. Endurance (aerobic) training, with light loads repeated many times, instead grows the muscle's stamina machinery: more mitochondria, denser capillaries, and better fat-burning, alongside a heart that pumps more per beat — changes that raise aerobic capacity but add little raw strength.

In rehabilitation this is why the goal must drive the method. A patient who cannot rise from the toilet needs strength training; one who tires crossing the ward needs endurance work; many need both, addressed deliberately rather than hoping one fixes the other. It also tempers expectations: the two types of adaptation can even interfere a little when pushed hard at once, and neither happens overnight — visible muscle growth in particular takes weeks of consistent, progressive effort.

Two weeks into rehab a patient is noticeably stronger at standing up, yet his thighs look the same size. His gains so far are neural — the nervous system learning to use the muscle he already has — with true muscle growth still weeks away.

Early strength gains are the nervous system getting smarter, not the muscle getting bigger.

A common mistake is assuming aerobic exercise will restore strength, or that heavy lifting will build stamina; because adaptations are specific, the wrong choice can waste weeks of a patient's limited rehabilitation time.
