muscle hypertrophy and atrophy

Muscle is not a fixed possession; it is constantly remodelled according to how much you use it. Use it hard and it grows; neglect it and it shrinks. Those two opposite directions are hypertrophy (growth) and atrophy (wasting), and they are governed by a continuous tug-of-war between building muscle protein up and breaking it down.

Hypertrophy is the enlargement of muscle fibres — they thicken as the body adds more contractile protein in response to the repeated mechanical stress of resistance training and adequate nourishment. Atrophy is the reverse: when a muscle is unloaded — by bed rest, a limb in a cast, a nerve injury, or simple disuse — the breakdown of protein outpaces its rebuilding and the fibres thin and weaken. Disuse atrophy is strikingly fast: meaningful loss of muscle size and strength begins within days of immobilization and can reach roughly one to a few percent per day in the early period, with anti-gravity muscles like the quadriceps hit hardest.

In rehabilitation this dual reality is both the threat and the opportunity. The threat is that every day a limb or a patient goes unused, muscle melts away — which is the core argument for early mobilization and for keeping joints moving even when a patient cannot yet exercise hard. The opportunity is that the same plasticity that allows atrophy allows recovery: with progressive loading, atrophied muscle can regrow. But it is honest to say that rebuilding is slower than losing, and that some atrophy — for example after long-standing nerve damage — may only partly reverse.

Six weeks in a cast for a broken ankle, and a patient is shocked to see how thin and weak that calf has become next to the other leg. The disuse atrophy is real, but with graded loading over the following months the muscle steadily rebuilds.

Muscle melts away within days of disuse but rebuilds only over weeks to months — losing is fast, regaining is slow.

Disuse atrophy (a starved-but-healthy muscle that can regrow) is not the same as atrophy from a cut nerve (denervation), where regrowth depends on whether the nerve recovers — an important distinction for setting realistic goals.
