The six-minute walk test measures something the lab numbers cannot: how far you can actually go. The patient walks back and forth along a flat, measured corridor at their own chosen pace for six minutes, and the total distance covered is the headline result — a simple, real-world snapshot of functional exercise capacity.

Along the way, oxygen saturation is tracked with a pulse oximeter to catch any exertional desaturation, and breathlessness is rated before and after. Together, distance walked and oxygen drop reflect the combined performance of the lungs, heart, circulation and muscles under everyday demand.

Its strength is practicality and its sensitivity to change — it is widely used to follow chronic lung and heart disease, to gauge the need for supplemental oxygen, to assess fitness for transplant, and to measure the benefit of pulmonary rehabilitation. Its limitation is that it is non-specific: a short walk tells you the patient is limited, but not why, so it complements rather than replaces formal lung function and cardiac testing.

A patient with interstitial lung disease walks 380 metres but his oxygen saturation falls from 95 to 86 percent — significant exertional desaturation supporting the case for ambulatory oxygen.

Distance walked and oxygen drop together gauge real-world capacity.

Results depend heavily on how the test is conducted — corridor length, encouragement and a practice walk all affect the distance — so standardised technique is essential for fair comparison over time.
