Real life rarely moves a joint in one clean direction. Reaching to put a book on a high shelf, your arm rises, rotates, and turns the palm all at once — a diagonal, spiralling motion. Proprioceptive neuromuscular facilitation is a hands-on technique built around exactly these natural diagonal and spiral patterns, the idea being to harness the body's own reflexes and sensors to make weak or uncoordinated muscles fire more strongly.

The "proprioceptive" part means it leans on the body's position-and-tension sensors; "facilitation" means making muscle activity easier to switch on. A therapist applies resistance, stretch, and specific verbal and hand cues along these diagonal patterns to recruit muscle effort. PNF is also widely used for flexibility, through clever tricks of the nervous system: in the contract-relax method, you contract a tight muscle against resistance and then it relaxes more deeply, allowing a greater stretch; in hold-relax and contract-relax-antagonist-contract, contracting the opposing muscle helps the tight one let go. Other named techniques — rhythmic initiation, slow reversals — coax movement out of a reluctant limb.

In the clinic PNF turns up most for stretching (where contract-relax is genuinely effective at gaining range) and as a hands-on way to facilitate movement in neurological patients. Like Bobath, its original neurophysiological rationale is dated, and as a whole-system motor-relearning approach it has not clearly outperformed simpler task practice. But its stretching techniques have stood the test of time, and its diagonal patterns remain a useful vocabulary for guiding functional, multi-joint movement.

To gain hamstring length, the therapist raises the leg to its stretch limit, has the patient push the leg down against firm resistance for a few seconds, and then — as the muscle relaxes — eases the leg further up. The contract-relax trick wins a little more range each round.

Contracting a muscle just before stretching it lets it relax into a deeper length.

PNF's contract-relax stretching is well supported, but its old explanation (that the trick works purely through a reflex called autogenic inhibition) is now doubted — the technique works even where that reflex story does not fully apply.
