How does the body decide how to move? Over the last century, scientists have offered very different stories, and those stories quietly shape how therapists teach a stroke survivor to reach or walk. If you believe movement is built from reflexes, you train by triggering reflexes; if you believe it emerges from the whole person interacting with the task and the room, you train in real tasks. The theory is not just academic — it changes what happens in the gym.
Three big ideas, roughly in order of age, are worth knowing. The reflex theory said movement is a chain of reflexes — one response triggers the next; useful but unable to explain movements made without any sensory trigger. The hierarchical theory said the brain commands from the top down, higher centres bossing lower ones; useful for understanding upper versus lower motor neuron signs, but too one-directional. The modern dynamical-systems or systems model says movement emerges from many interacting players at once — the nervous system, the muscles and bones, the specific task, and the environment — with no single boss; how you reach depends on the cup, the table height, your fatigue, and the floor, all together.
The systems model now guides mainstream rehabilitation. It is the reason therapy has shifted from drilling isolated muscle movements on a mat toward task-oriented training in realistic settings — practising actually drinking from a real cup at a real table, because the task and environment are part of the control problem, not a distraction from it. The honest point is that no single theory is the whole truth; each captures part of how a richly layered, looping system produces movement, and good clinicians borrow from all three.
Guided by the systems model, a therapist does not drill 'lift the arm' on a table; she has the patient practise reaching for cups at different heights and distances, because changing the task and surroundings is what actually reshapes the movement.
The systems model is why modern therapy practises whole, meaningful tasks.
These are competing frameworks, not proven laws. The reflex and hierarchical theories are not 'wrong' so much as incomplete; the systems model is favoured today but still simplifies an extraordinarily complex process.
