biosimilar
A biosimilar is a follow-on version of an approved biologic that is highly similar to the original, with no clinically meaningful differences in how it works or how safe it is. It is the biologic world's answer to the generic drug — but it is not a generic, because you cannot make an exact copy of something grown in living cells. Think of it as a faithful cover of a song: recognizably the same piece, performed so closely that listeners cannot tell the difference, even though it is not the identical recording.
The reason an exact copy is impossible is that biologics are large, complex proteins produced by living cells, and even the original maker's own batches vary slightly from one to the next. A different company, using its own cell line and process, cannot reproduce the molecule atom-for-atom. So a biosimilar is held to a standard of high similarity rather than identity: it must match the reference product closely on structure, biological activity, purity, and the variants present.
Proving biosimilarity is a careful, evidence-heavy exercise. Developers run extensive analytical comparisons of structure and function, then targeted laboratory and clinical studies to confirm the small remaining differences do not matter for patients. The formulation may differ — a different buffer, a different presentation — as long as the product behaves equivalently; the goal is the same effect in the patient, not an identical ingredient list.
An honest caveat: biosimilar is a rigorous regulatory designation, not a loose word for similar. Products that have not gone through this comparability pathway are not biosimilars, and a biosimilar is generally interchangeable with the reference product only to the extent and in the ways its approval and local regulations allow.
After a blockbuster antibody loses patent protection, several companies may launch biosimilar versions that match its structure and effect closely enough to be used in place of it, often at lower cost.
