biologic
A biologic is a medicine that is grown rather than chemically synthesized. Instead of building a small molecule atom by atom in a reaction flask, manufacturers coax living cells — bacteria, yeast, or cultured mammalian cells — into producing the active substance, much like brewing rather than baking. The result is usually a large, delicate molecule such as a protein, an antibody, a hormone, or the genetic material of a vaccine.
Because they come from living systems, biologics are enormous and structurally complex compared with conventional drugs. An aspirin molecule has 21 atoms; a therapeutic antibody has tens of thousands and folds into a precise three-dimensional shape decorated with sugars. That shape is the medicine — break the fold or alter the sugars and the activity, and sometimes the safety, change. This is why a biologic is often described as defined by its process: small changes in the cells or purification can change the product.
Their size and fragility dictate everything about how they are formulated and used. Most biologics cannot survive the stomach, so they are given by injection or infusion rather than as tablets. They are sensitive to heat, freezing, light, shaking, and surfaces, so they need carefully chosen excipients and tight temperature control. They are also expensive and slow to make, which shapes their pricing and the rise of follow-on versions.
An honest caveat: biologic is a broad regulatory and practical category, not a single chemical class. It spans simple peptides, complex monoclonal antibodies, cell and gene therapies, blood products, and vaccines, and the rules and challenges differ greatly across this range. Calling something a biologic tells you how it was made and roughly how careful you must be, not exactly what it is.
Insulin, the antibody trastuzumab, and the hepatitis B vaccine are all biologics, even though one is a small hormone, one is a large antibody, and one is an immunizing antigen.
One label, very different molecules.
Contrast with a conventional small-molecule drug, which is chemically synthesized, well-defined, and usually stable and orally available. The distinction is practical: biologics demand cold storage, injection, and tighter control of subtle structure.
