pharmacogenomics

Pharmacogenomics is the study of how a person's genes shape the way they respond to drugs. We have all noticed that the same medicine at the same dose can be a perfect fit for one person, useless for another, and harmful to a third. A large part of that variation is written in our DNA, and this field reads that script to predict who will benefit and who is at risk.

Genes influence response in two main ways. Some affect pharmacokinetics, coding for the enzymes and transporters that absorb, break down, and remove drugs; a common variant in a liver enzyme can make someone a slow metabolizer who accumulates a drug to toxic levels, or a fast metabolizer who clears it before it can work. Other genes affect pharmacodynamics, altering the drug's target itself, such as a receptor or an immune molecule, and changing how strongly the drug acts.

In practice, this knowledge lets clinicians test a patient's genotype before prescribing, then adjust the choice of drug or its dose accordingly. Familiar examples include screening certain enzymes before giving particular blood thinners, antidepressants, or chemotherapy, and testing for immune-system gene variants that predict dangerous skin reactions to specific drugs.

Genes are only one piece of the puzzle. Age, kidney and liver function, other medicines, diet, and disease all sway drug response too, so a gene test rarely dictates the answer by itself. For many drugs the genetic links are still incompletely understood or too weak to act on, and the field is most useful where a clear, well-validated gene strongly predicts benefit or harm.

People who carry two slow-acting copies of the liver enzyme CYP2C19 break down the antiplatelet drug clopidogrel poorly, so it protects them less, which is one reason genotype testing is sometimes used before prescribing it.

A single inherited enzyme variant can blunt or amplify a drug's effect.

Pharmacogenetics traditionally meant the effect of single genes, while pharmacogenomics scans the whole genome; the two terms are now often used interchangeably.
