Personalized medicine is the practice of tailoring the choice and dose of a treatment to the individual in front of you, rather than handing everyone the same standard prescription. The old model gives the average patient the average drug at the average dose; the personalized model asks what makes this particular patient different, and uses that to pick a treatment more likely to help them and less likely to harm them.
The information used to personalize can come from many sources: a patient's genes, the molecular profile of their tumor or infection, blood markers, organ function, age, weight, other illnesses, and the other drugs they take. In its strongest form, a test identifies a feature that predicts response, and the drug is then given only to patients who carry it, sometimes paired with a companion diagnostic test approved alongside the drug.
Cancer is the clearest showcase. Tumors are increasingly profiled for specific mutations, and a targeted drug is chosen to match, so that a therapy works for one patient's tumor and would be useless against another's. Beyond genetics, the broader idea also covers adjusting doses to kidney function or steering away from a drug a patient is genetically prone to react badly to.
Despite the appealing promise, personalized medicine is not magic and not yet universal. For many conditions we still lack reliable markers to personalize by, predictive tests are imperfect and can mislead, and targeted treatments can be costly and may lose effectiveness as disease evolves. It refines, rather than replaces, the population-based evidence from clinical trials.
A lung-cancer patient whose tumor carries a specific EGFR mutation is given an EGFR-targeted tyrosine kinase inhibitor that would be ineffective in a patient without that mutation.
Matching a targeted drug to a tumor's molecular profile is personalized medicine in action.
A companion diagnostic is a test approved to be used alongside a specific drug, identifying the patients in whom that drug is expected to work.
