off-target effects

An off-target effect is when a genome editor cuts or changes DNA somewhere other than the place you intended. It is the central reliability worry of editing: the tool is precise, but not perfectly precise, and a stray edit in the wrong gene could be harmless, or could disable a tumour-suppressor gene and start trouble. Understanding why off-targets happen is essential to using these tools honestly.

The root cause for CRISPR is the very thing that makes it easy: targeting by base pairing. The guide RNA finds its site by matching about 20 letters, but base pairing tolerates a few mismatches, so a sequence elsewhere in the genome that is similar — say it differs by only one or two letters — can also be recognised and cut, especially if the differences fall in the less-critical part of the match. With three billion letters to choose from, near-matches are common. Older tools like zinc-finger nucleases and TALENs had their own off-target cutting too. Researchers reduce the risk by designing guides against sequences that are as unique as possible, using higher-fidelity engineered Cas9 variants that are choosier, delivering the editor only briefly, and then sequencing to search for unintended edits.

This matters enormously, especially for any medical use: an off-target edit cannot be undone, and in a treated patient it could in principle cause cancer or other harm. It is also a reason to be skeptical of claims that an edit was 'perfectly clean' — proving the absence of off-targets across an entire genome is genuinely hard, and the more cells you treat the more chances there are for a rare off-target to appear somewhere. Off-target effects are not a sign the tool is broken; they are an inherent limit that careful design manages but never entirely eliminates.

A guide meant for gene A, sequence ...GACCTGAAGTCCATGGCTAA, may also partly match ...GACCTGAAGTCCATGGCTTA in gene B, differing by one letter — and Cas9 may cut gene B too.

A near-match elsewhere can be edited by mistake.

Claiming an edit is completely free of off-targets is hard to prove and easy to overstate. Off-target risk is intrinsic to sequence-matching tools; good practice manages and measures it rather than pretending it is zero.
