If Cas9 is a pair of scissors, the guide RNA is the address label that tells the scissors where to go. Cas9 by itself is sequence-blind — it has no built-in idea of which DNA to cut. The guide RNA supplies that knowledge, and the elegant part is that it points the scissors not by complicated protein shape-matching but by simple base pairing, the same A-to-T, G-to-C rule that holds DNA together.
A guide RNA carries a short stretch — about 20 nucleotides — called the spacer, whose sequence you choose to match your target. Inside the cell, Cas9 loaded with the guide scans the DNA; wherever it finds a PAM, it pries the double helix open and offers the guide's spacer to one DNA strand. If the bases line up — guide RNA pairing with target DNA along that 20-letter window — Cas9 locks down and cuts; if they mismatch, it lets go and moves on. In practice the natural system used two RNAs, but researchers fused them into one convenient molecule called a single guide RNA (sgRNA): part of it base-pairs the target, and the rest folds into a handle that Cas9 grips.
The guide RNA is exactly why CRISPR is so easy to reprogram. To aim the editor at a new gene, you do not redesign any protein — you simply synthesise a guide with a new 20-letter spacer, a quick and cheap step. This is the whole reason CRISPR overtook zinc-finger nucleases and TALENs. The honest caveat: base pairing tolerates a few mismatches, so a guide can also pair with similar-but-wrong sequences elsewhere, which is the root cause of off-target editing. Good guide design tries to pick spacers unique in the genome to keep that risk low.

A guide does not bind only its perfect match. Because base pairing tolerates some mismatches, a guide can partially pair with similar sequences, so off-target cutting is a property of the targeting principle itself, not just sloppy design.
