immunomodulator

An immunomodulator is a drug that adjusts the immune system rather than simply switching it off or on. Think of it as a dimmer dial rather than a plain on/off switch: depending on the drug and the situation, it can turn immune activity up to fight cancer or infection, or down to calm overactive inflammation. The unifying idea is fine adjustment, not blanket suppression or stimulation.

The category is broad. Some immunomodulators boost immunity, such as interferons and interleukins, or the immune checkpoint inhibitors that re-awaken T cells against tumours. Others restrain it, like many drugs used in autoimmune disease. A famous group, the immunomodulatory imide drugs (thalidomide, lenalidomide), reshape immune and tumour-environment signalling and are mainstays in multiple myeloma. Because immunomodulator is a function-based label, drugs with very different chemistry can share it.

The practical caution is that nudging the immune system is rarely purely beneficial. Boosting it can trigger excessive inflammation or autoimmune-like side effects, while damping it raises infection risk. The thalidomide story is also a permanent warning: it is a powerful teratogen, requiring strict pregnancy-prevention programmes wherever it is used.

Immunomodulator is the umbrella term; immunosuppressants (always down) and immunostimulants (always up) are its two one-directional subsets, while checkpoint inhibitors are activating immunomodulators used in cancer.
