teratogen

A teratogen is something that, when a pregnant person is exposed to it, can disturb the building of the developing fetus and cause birth defects. The word comes from the Greek for 'monster-forming'; in practice it means an agent that interferes with the delicate, scheduled construction work of an embryo.

Timing is everything. The most vulnerable window is organogenesis, roughly weeks three to eight after conception, when major organs are being laid down; a teratogen acting then can cause structural malformations of the heart, limbs, face, or neural tube. Earlier exposure tends to be 'all or nothing' (the embryo survives intact or is lost), while later exposure more often impairs growth and function rather than causing gross structural defects.

Teratogens include certain drugs (thalidomide, isotretinoin, valproate, warfarin, some ACE inhibitors), infections (rubella, Zika), alcohol, radiation, and metabolic disturbances. The effect usually depends on dose, gestational timing, and the genetic susceptibility of both mother and fetus, which is why the same exposure does not harm every pregnancy equally.

Because controlled experiments in pregnant humans are unethical, much teratogenicity is recognised only after the fact — the thalidomide tragedy of the late 1950s and early 1960s reshaped drug regulation worldwide. This is why drugs are assigned pregnancy risk categories and why so many carry the cautious default advice to avoid use in pregnancy unless clearly necessary.

Isotretinoin, used for severe acne, is so reliably teratogenic that prescribing it requires strict pregnancy-prevention programmes with mandatory contraception and pregnancy testing before, during, and after treatment.

Some drugs demand contraception as a condition of use.
