Glucose-stimulated insulin secretion is the elegant chain of events by which a beta cell turns a rise in blood sugar into a release of insulin — the central reflex of the endocrine pancreas. In plain terms, the cell tastes more sugar, burns it for energy, and uses that energy spike as the signal to dump insulin.

Step by step: glucose enters through GLUT2 and is phosphorylated by glucokinase, the rate-limiting glucose sensor; metabolism raises the ATP-to-ADP ratio; this closes ATP-sensitive potassium channels, depolarizing the membrane; voltage-gated calcium channels open; and the calcium influx triggers insulin granule exocytosis. Glucokinase sets the threshold at which this whole process turns on.

This pathway is the target of major drugs and a hotspot for disease. Sulfonylureas force the potassium channels shut to boost insulin; certain forms of MODY arise from glucokinase mutations that reset the sensor; and amplifying signals from incretins like GLP-1 enhance secretion only when glucose is already high, which is why they rarely cause hypoglycemia.
