Post-trial device access and abandonment

A structural ethics problem specific to investigational neural implants: when a study ends, a sponsor pivots, or a device line is discontinued, patients who benefited may lose programming support, replacement parts, or battery service, and can face explantation of a device they experience as part of themselves.

Documented cases — including participants who relapsed or lost a valued restored capacity when an experimental system was withdrawn — have moved post-trial obligations, long-term maintenance, and continuity of care from afterthoughts to recognized duties in neurotechnology research ethics. In psychiatry the stakes are especially high because the therapy targets mood and identity, and the problem foreshadows broader equity and access questions as psychiatric neuromodulation moves from small trials toward wider care.
