Closed-loop DBS for depression
Closed-loop (adaptive or responsive) deep-brain stimulation for depression senses an ongoing neural biomarker of pathological state and delivers stimulation only when that state is detected, instead of stimulating continuously at fixed parameters (open-loop). The rationale is that mood disorders are episodic and state-dependent, so responsive delivery could in principle improve efficacy, reduce habituation and off-target side effects, and conserve battery.
As of the early 2020s this remains largely at the single-patient and small-cohort proof-of-concept stage. The landmark demonstration decoded a personalized limbic biomarker in one patient and triggered ventral-striatal stimulation only when that biomarker crossed threshold; no closed-loop psychiatric DBS system has regulatory approval or a completed efficacy randomized controlled trial. The read side (a reliable state detector) is the binding constraint, not the stimulation hardware.
Adaptive DBS is already used clinically for motor symptoms in Parkinson's disease, gated by a fast subcortical beta rhythm. Psychiatric closed loops are harder because the target state (mood) evolves over hours to days and lacks a comparably robust, generalizable biomarker.
