There is a chicken-and-egg trap in relearning to walk after a serious stroke or spinal injury: you cannot practise walking until your legs can hold your weight and step, but they may not get strong or coordinated enough to do that until you practise walking. Body-weight-supported treadmill training breaks the deadlock by letting you practise the act of walking before your legs can fully support you.
The setup is straightforward: the person is fitted into an overhead harness that takes some of their body weight off their legs, suspended above a moving treadmill. Relieved of part of the load, even weak legs can step, and one or two therapists (or a robotic device) help guide the legs through the stepping motion. As the legs grow stronger and more coordinated, the support is gradually reduced and the treadmill sped up, so they bear more and more of their own weight. The rationale draws on the spinal cord's own stepping circuitry (central pattern generators) and on giving the nervous system the chance to practise hundreds of step cycles — far more repetitions of the real movement than overground walking would allow so early.
It is a workhorse for early gait rehabilitation in stroke, spinal cord injury, and other neurological conditions, prized for delivering high-volume, repetitive, task-specific stepping safely, without the fear of falling. The honest verdict from research, though, is sobering: for many patients it is no better than an equivalent dose of plain, well-structured walking practice on the ground. It is a useful tool — especially when someone is too weak or too unsteady to walk overground at all — rather than a magic device, and the support must be weaned, or the body never learns to carry itself.
Weeks after an incomplete spinal cord injury, a man hangs in a harness that carries 40% of his weight while the treadmill rolls slowly and therapists guide each leg through the step. As his stepping improves, they dial the support down to 20%, then 10%.
The harness lets weak legs rehearse stepping; the support is then steadily withdrawn.
Trials repeatedly show it is generally no more effective than the same amount of structured walking practice on the ground — the benefit is mostly the safe, high-repetition practice it enables, not the treadmill itself.
