Step into a chest-deep pool and your body suddenly feels light — you could lift a leg that aches to lift on land, or take a step that your knees would protest on dry ground. That magic is buoyancy, and aquatic therapy is the deliberate use of a pool as a place to exercise, harnessing the special physics of water to do rehabilitation that would be too painful or too hard on land.
Water offers a unique combination of properties. Buoyancy lifts much of the body's weight, unloading painful or weak joints so that someone can stand, step, or strengthen with far less stress — a person who cannot bear full weight on a healing leg can still practise walking patterns in the pool. Water's resistance is gentle yet ever-present, so any movement is a light strengthening exercise, and warm water relaxes muscles and eases pain. The same buoyancy that helps also cushions falls, so balance can be challenged safely. Therapists design exercises in the water exactly as they would on land — range of motion, strengthening, gait, balance — but with the load and the safety net that water provides.
Aquatic therapy is valuable for arthritis, after joint surgery, for obesity, for chronic pain, and for people too weak or unsteady for much land-based work. Its great limitation is that it is a stepping stone, not a destination: the body must eventually relearn to function under full gravity on dry land, so skills gained in the pool have to be transferred out of it. It also requires a suitable pool and excludes people for whom water is unsafe — uncontrolled seizures, certain wounds or infections, severe incontinence, or a fear of water.

The pool is a bridge, not the goal: gains made with buoyancy must be carried back to full-gravity land. And water is unsafe for some — uncontrolled seizures, open or infected wounds, and severe incontinence among them.
