Some stroke survivors face a puzzle that looks at first like simple weakness but is actually stranger and deeper. Ask the person to wave goodbye on command and the hand fumbles uselessly — yet a minute later they wave goodbye perfectly to a leaving visitor without thinking. The muscles work and the strength is there; what is broken is the link between the idea of a movement and the program that carries it out. This is apraxia: a loss of the 'how-to' for skilled, learned actions.

Apraxia comes from damage (usually in the left brain) to the regions that store and sequence the recipes for purposeful movement. The classic clue is the gap between automatic and deliberate action — a person may be unable to demonstrate brushing their teeth when asked, but does it fluently each morning out of habit. They may use the wrong sequence, fumble with familiar tools, get the timing or the spatial path wrong, or use a body part as the object (running a finger like a toothbrush). It is not weakness, not numbness, and not a failure to understand the request — and it often hides behind aphasia, making it easy to miss.

In rehabilitation apraxia matters because it can quietly sabotage relearning of everyday tasks: the person 'can' move but cannot organise the move on demand, which frustrates both patient and family who may think they are not trying. Occupational therapists work around it by using real objects in real contexts (which trigger the automatic version), breaking tasks into steps, giving consistent cues, and practising the same routine the same way until it runs on autopilot. The honest point is that recognising apraxia changes the whole approach — pushing harder on command-following often backfires; setting up the natural situation often works.

Asked to show how he would comb his hair, a man holds the comb upside down, brings it toward his ear, then stalls, looking baffled. Handed the comb in front of the bathroom mirror after breakfast, he combs his hair smoothly — the automatic routine survives even though the on-command version is broken.

The tell-tale of apraxia: automatic actions survive while the same action on command fails.

Apraxia is easily mistaken for not trying, laziness, or not understanding. It is none of those — and because it often coexists with aphasia, it is frequently missed unless someone watches the person attempt familiar tasks.
