The antimicrobial spectrum is the range of microbe types a drug can effectively kill or inhibit. Some drugs are like a precision rifle that hits only one kind of target (narrow-spectrum); others are like a wide net that catches many kinds at once (broad-spectrum).

For antibiotics, spectrum is often described by which bacteria are covered: Gram-positive, Gram-negative, both, plus extras like anaerobes or atypical organisms. The spectrum is set by the drug's mechanism and by whether it can actually reach its target — for instance, the extra outer membrane of Gram-negative bacteria keeps many drugs out, narrowing their reach.

Choosing the right spectrum is a real clinical trade-off. Broad-spectrum drugs are invaluable when the culprit is unknown or an infection is severe, but they also wipe out helpful bacteria and apply pressure that breeds resistance across many species. Good practice is to start broad if needed, then narrow the spectrum once the lab identifies the actual pathogen — a strategy called de-escalation.

Vancomycin has a narrow spectrum aimed mainly at Gram-positive bacteria, whereas a carbapenem covers a very broad range including many Gram-negatives.

Spectrum width guides whether to aim narrow or cast wide.

Broad-spectrum is not automatically better: it disrupts more of the body's normal microbiome and raises the risk of complications like Clostridioides difficile colitis.
