zero-order release

Zero-order release is the formulator's holy grail: the same amount of drug comes out every minute, like a metronome ticking at a steady beat no matter how much time has passed. The rate does not care how much drug is left in the dosage form.

Mathematically, the amount released is a straight line against time — if you plot cumulative drug released, you get a constant upward slope. This contrasts with first-order release, where the rate fades as the dose runs low. Constant input means the blood level can be held flat inside the therapeutic window, with no big peaks (toxicity risk) or troughs (loss of effect).

Achieving true zero order requires a constant driving force. Osmotic pumps do this best, because osmotic pressure stays steady until the drug is nearly exhausted; saturated-core reservoir systems and certain swelling or erosion-balanced matrices also approximate it. In practice perfect zero order is rare — most products only follow it over part of the release.

Zero order describes a constant rate; do not confuse it with a flat blood-level curve, which also depends on how fast the body clears the drug.
