volume regulation
Volume regulation is how the body keeps the right amount of blood circulating — enough to fill the vessels and supply every organ, but not so much that the heart is overloaded. Think of it as keeping a water tank at the right level so the pumps stay primed without overflowing.
The body cannot measure blood volume directly, so it reads pressure and stretch at sentinel points: baroreceptors in the arteries, stretch receptors in the heart and large veins, and the kidney sensing how much blood reaches it. When effective volume falls, the renin-angiotensin-aldosterone system and vasopressin switch on to retain salt and water and tighten vessels. When volume is high, natriuretic peptides from the stretched heart promote salt and water loss. Because sodium holds water in the circulation, volume is controlled mainly by controlling body sodium.
A subtle point is that the body defends “effective” circulating volume — the amount actually filling the arteries — not the total fluid present. In heart failure or liver cirrhosis the body may sense low effective volume and avidly retain salt and water even though the patient is visibly overloaded with fluid, which is why swelling and salt retention can coexist with a sense of underfilling.
