Tmax
Tmax is the clock reading at the top of the curve — the moment a drug hits its highest blood concentration after a dose. If you took a tablet and the drug peaked in your blood two hours later, the Tmax is two hours. It tells you not how high the drug got, but when it got there.
Tmax reflects mainly how fast a drug is absorbed relative to how fast it is eliminated. A quickly absorbed drug peaks early, giving a short Tmax; slow absorption, food in the stomach, or a sustained-release coating pushes the peak later. Up to Tmax, absorption outpaces elimination; after Tmax, elimination takes over and the concentration falls.
Practically, Tmax matters when timing matters: a fast Tmax suits a rescue medicine needed to work within minutes, while a deliberately delayed Tmax can be useful for a bedtime dose meant to act overnight. Along with Cmax, it is a standard descriptor of how a formulation behaves, especially when comparing products for bioequivalence.
Taking a drug with a fatty meal can delay gastric emptying and push its Tmax from one hour to three or more, slowing the onset of effect even if the total amount absorbed is unchanged.
Food can delay Tmax without changing total absorption.
Tmax indicates the speed of absorption, not the amount absorbed; a drug can be fully absorbed yet have a late Tmax simply because absorption is slow and steady.
