Timed Up and Go

Some of the most useful tests in medicine are also the simplest. The Timed Up and Go, almost always called the TUG, is one of them: you simply time how long it takes a person to stand up from a chair, walk three metres, turn around, walk back, and sit down again. A stopwatch and a chair are all you need, and the whole thing takes less than a minute.

Although it gives just one number — the seconds taken — that number packs in a lot, because the task strings together several real-world skills: rising from sitting (which needs leg strength), walking (which needs gait and endurance), turning (which challenges balance), and sitting back down safely (which needs control). The person uses whatever walking aid they normally use, and the test is usually done at a comfortable pace. Broadly, faster times reflect better mobility, and longer times — many clinicians watch for around 12 to 14 seconds or more in older adults — flag a person who may be at higher risk of falling and worth a closer look. Watching how the person moves during the test is as informative as the clock.

The TUG matters because it is a quick, repeatable, real-life snapshot of mobility that any clinician can do anywhere, and it is sensitive to change, so improvement after surgery or therapy shows up as a faster time. Its honest limits are that a single cut-off does not fit everyone — age, height, and the type of walking aid all affect the time — and that the seconds alone do not say why someone is slow. It is best read together with the observation of how the movement looked, not as a number in isolation.

Before a knee replacement, a man completes the TUG in 22 seconds, rising slowly and gripping the chair. Three months after surgery and therapy he does it in 11 seconds without using his arms. The halved time, with the smoother movement, captures his recovery in one quick measure.

No single TUG cut-off fits everyone: time is affected by age, height, and walking aid, and the seconds alone never say why a person is slow — watching how they move during the test matters just as much.
