Target engagement is direct evidence that a drug actually reaches and binds its intended target inside living cells or tissue, not just in a test tube. It answers a deceptively simple question: did the drug get to where it was supposed to go and grab what it was supposed to grab? Without this proof, it is hard to interpret whether a drug works or fails.

Researchers demonstrate target engagement using assays that report binding in a real biological setting, such as measuring how the drug changes the target's stability, displaces a labeled probe, or shifts a marker that only moves when the target is occupied. The goal is to confirm a physical drug-target encounter at the relevant place and dose, distinguishing it from indirect or off-target effects.

Target engagement bridges a crucial gap between a compound binding in a purified assay and a drug producing an effect in an organism. If a drug shows the desired benefit but no engagement, the effect may come from something other than the intended target. The honest caveat is that engagement alone is not enough: a drug can bind its target faithfully and still fail to help if the target was the wrong one or occupancy was too brief.
