A stimuli-responsive system is like a door that stays shut until it senses a particular signal, then opens. The carrier holds its drug until it meets a chosen trigger, and only then releases the dose, ideally right where and when it is needed.

Triggers can be internal, arising naturally at the disease site, such as the lower pH of a tumour or inside cellular compartments, raised levels of certain enzymes, or a more reducing chemical environment inside cells. Triggers can also be external and applied by the clinician, such as heat, light, ultrasound, or a magnetic field. The carrier is engineered so that the trigger changes its structure, for example a polymer that swells, dissolves, or breaks a chemical bond, thereby unloading the drug.

The appeal is precise control over where and when release happens, which could reduce side effects and tailor therapy. The reality is harder: internal triggers are often only modestly different between diseased and healthy tissue, externally applied stimuli must reach deep tissue accurately, and the materials must be safe and reproducible. Most stimuli-responsive systems remain experimental, with relatively few reaching patients so far.
