The spectrum of activity is simply the range of microbes a given antimicrobial can effectively treat. Some drugs are like a precision tool that fits one bolt; others are like an adjustable wrench that grips many. A narrow-spectrum drug hits only a limited set of organisms, while a broad-spectrum drug covers a wide variety, including both Gram-positive and Gram-negative bacteria.

Spectrum reflects mechanism and access. A drug whose target exists only in certain bacteria, or that cannot penetrate the outer membrane of Gram-negative organisms, will be narrow by nature. Clinicians often start with a broad-spectrum agent for a serious infection of unknown cause, then de-escalate to a narrower drug once cultures identify the organism, a practice called streamlining.

Broad is not better. Broad-spectrum drugs collateral-damage the body's beneficial flora, fuel resistance across many species at once, and raise the risk of secondary infections such as Clostridioides difficile colitis and thrush. The guiding principle is to use the narrowest agent that reliably covers the likely pathogen, reserving broad-spectrum firepower for when it is genuinely warranted.

A patient with sepsis is started on broad-spectrum piperacillin-tazobactam, then switched to narrow-spectrum flucloxacillin once the culture grows only Staphylococcus aureus.

Start broad when the cause is unknown, then narrow once the organism is identified.

Spectrum can shift over time: a drug once considered broad against a pathogen may become effectively narrow as resistance erodes its coverage in a region.
