side effect

A side effect is an effect of a drug that comes alongside the one you actually wanted — like buying a tool that does the job but also makes a noise you did not ask for. It is a secondary action, usually undesirable, that arises at normal doses and is generally predictable from how the drug works.

Most side effects trace back to the same pharmacology that gives the benefit. A drug rarely hits only one target perfectly: it may also bind related receptors elsewhere in the body, or its desired action in one organ may have downstream consequences in another. An antihistamine that blocks histamine to relieve allergy also blocks it in the brain, producing drowsiness; an opioid that relieves pain also slows the gut, causing constipation.

Importantly, a side effect is not always bad. Because it is just a 'non-primary' action, an unwanted effect in one context can become the desired effect in another. The drowsiness of certain antihistamines is repurposed as a sleep aid, and minoxidil's blood-pressure effect was overshadowed by the hair growth it caused.

Clinicians and patients tolerate many mild side effects when the benefit is worth it, but a side effect that crosses into real harm at normal doses is then better described as an adverse drug reaction.

In everyday speech 'side effect' and 'adverse reaction' are used interchangeably, but pharmacologists keep them distinct: a side effect can be neutral or even beneficial, whereas an adverse reaction is by definition harmful.
