Some children's spines, during the fast growth of adolescence, begin to curve sideways into a C or S shape — a condition called scoliosis. If the curve is moderate and the child is still growing, doctors may try to keep it from getting worse with a brace. A scoliosis brace is a custom-moulded trunk orthosis designed not to walk or to immobilise, but to guide a growing spine and slow or stop a curve from progressing.

It works by the three-point pressure idea applied to a sideways curve. Built-in pads press against the rib cage and trunk at the apex of the curve, pushing it back toward the midline, while the brace holds the trunk above and below as the counter-forces. The aim is not to straighten the existing curve permanently — once growth is done, the brace cannot un-bend the spine — but to keep the curve from worsening during the years of rapid growth, so the child finishes growing with a smaller curve than they would otherwise have had. Designs include the Boston and Cheneau braces; some are worn many hours a day.

Bracing is one of the most evidence-tested orthotic treatments, and for the right adolescent with a moderate, progressing curve it genuinely reduces the chance of needing surgery — but only if it is actually worn for the prescribed hours, which is hard for a self-conscious teenager wearing a rigid jacket under clothes. Two honest points: a brace is a holding strategy during growth, not a cure that reverses the curve, and it has little role once growth is finished or for very large curves, where surgery may be the option discussed instead.
