A respiratory arousal is the brief, protective jolt of wakefulness the brain triggers when breathing during sleep runs into trouble. As an apnea, hypopnea, or simply a stretch of effortful breathing against a narrowed airway pushes oxygen down or carbon dioxide up, the brain briefly surfaces toward wakefulness, restoring muscle tone and airflow. The sleeper usually has no memory of it, yet sleep has been interrupted.

On the brain-wave recording, an arousal appears as an abrupt shift to faster, lighter activity lasting a few seconds. A specific category, the respiratory effort–related arousal, is reserved for an arousal driven by rising breathing effort against airway narrowing that does not meet the criteria for an apnea or hypopnea — too little airflow reduction or oxygen drop to qualify, yet enough to wake the brain.

Arousals are the engine of the daytime consequences of sleep-disordered breathing. Even when oxygen barely falls, repeated arousals fragment sleep, prevent deep restorative stages, and produce excessive daytime sleepiness, impaired concentration, and a stress response that can raise blood pressure. They are also central to upper airway resistance syndrome, where arousals dominate but apneas and major desaturations are absent.

Counting respiratory effort–related arousals alongside apneas and hypopneas gives the respiratory disturbance index, a fuller measure of sleep fragmentation than the apnea–hypopnea index alone. The honest caveat is that arousal scoring is somewhat subjective and detecting effort-related arousals reliably needs sensitive equipment, which is one reason simple home oximetry can miss this kind of sleep disruption.

A respiratory effort–related arousal (RERA) fragments sleep through arousal even without a qualifying airflow or oxygen change. Adding RERAs to apneas and hypopneas gives the respiratory disturbance index. Detecting RERAs reliably needs brain-wave and effort sensors.
