Every prosthesis has to attach to the body, and that attachment is the most important — and most troublesome — part of the whole device. The socket is the custom-moulded cup that the residual limb fits into; the suspension is whatever holds that socket reliably on the limb so it does not slip off when the leg swings or the arm lifts. Get these right and the rest of the prosthesis works; get them wrong and the fanciest foot or hand is useless.

The socket is shaped individually for each person, because it must spread the load of body weight onto the areas of the limb that tolerate pressure well and relieve the areas that do not (a bony edge, a nerve, a scar). A socket that fits is the single biggest factor in comfort and skin health. Suspension — keeping it on — comes in several forms: suction, where air is pushed out so atmospheric pressure holds the limb in like a plunger; a pin lock, where a pin on the end of the liner clicks into a catch at the bottom of the socket; straps and belts that buckle around the body; and elevated vacuum, where a small pump actively maintains a vacuum, gripping the limb firmly and even helping to stabilise its volume.

Socket and suspension problems are the daily bread of amputee clinics. If suspension is loose, the limb pistons up and down inside the socket with each step, shearing the skin and causing blisters and sores. If the socket presses where it should not, the skin breaks down. Because the limb's volume changes through the day and over months, fit is never permanently solved — amputees add or remove socks to fine-tune the fit, and the socket is periodically remade. The blunt truth is that comfort lives or dies at this interface, no matter how advanced the components below it.

A patient complains of blisters at the end of his limb; on examination the socket is slightly loose, so the limb pistons up and down with each step. Adding one more limb sock tightens the fit, stops the pistoning, and the blisters heal.

Loose suspension lets the limb piston and shear the skin; the fix is often as simple as adjusting sock ply.

No prosthetic foot, knee, or hand can compensate for a poorly fitting socket; the socket-limb interface is where comfort, skin health, and whether the device gets worn at all are truly decided.
