A prodrug is a medicine in disguise. It is given in an inactive form and only becomes the real, active drug after the body chemically alters it — much like a seed that does nothing until soil and water turn it into a plant. The disguise is deliberate: it is engineered to fix some problem the active drug has on its own.
Chemically, a prodrug is the active molecule fitted with a removable masking group, often an ester, carbamate, or phosphate. Once inside the body, enzymes such as esterases or cytochrome P450s, or simple chemical conditions, cleave that group and release the active species. Designers use this trick to improve poor oral absorption, mask a bitter taste, prolong action, or target the drug to a particular tissue.
The catch is that activation must be reliable. If the converting enzyme varies a lot between people, or if conversion is incomplete or too slow, the delivered dose of active drug becomes unpredictable. A prodrug also adds a metabolism step that must be characterized for safety, since the cleaved masking group itself becomes a metabolite that has to be shown to be harmless.
Oseltamivir (Tamiflu) is an ester prodrug; liver esterases hydrolyze it to the active carboxylate, which gives much better oral absorption than dosing the active form directly.
