A prefilled syringe is a syringe that arrives already loaded with one exact dose of medicine, sealed and sterile, ready to inject. Instead of drawing a drug from a vial — measuring, expelling air, risking spills or contamination — the user simply uncaps and injects. It is the difference between scooping coffee, grinding, and brewing versus a single-serve pod: the measuring and packaging are done in advance for convenience, accuracy, and safety.

Prefilled syringes are especially common for biologics such as antibodies and for vaccines, because these are given by injection, often by patients at home. They reduce dosing errors and the wastage that comes from extracting the last drops of a vial, support patient compliance for chronic self-injected therapies, and underpin autoinjector pens, which are essentially a prefilled syringe inside a spring-loaded device that hides the needle and delivers the dose at the press of a button.

From a formulation and packaging standpoint the syringe is part of the product, not just a delivery tool. The drug now sits in long-term contact with the syringe barrel (often glass or polymer), the rubber plunger, and any silicone oil used to make the plunger glide; each can interact with a sensitive protein, leach substances, or shed silicone droplets that seed aggregation. So the container-closure system must be qualified together with the formulation, and compatibility and particle testing become central.

An honest caveat: convenience comes with constraints. A prefilled syringe locks in a single fixed volume and dose and cannot easily be adjusted, it is more complex and costly to manufacture sterile, and its materials can subtly affect a delicate biologic — which is why such a simple-looking device demands extensive development and testing.

Silicone oil, used to let the plunger slide smoothly, can shed microscopic droplets into the liquid that act as seeds for protein aggregation — a good example of how the container is not a passive bystander to a biologic.
