A placebo is a treatment with no active drug in it, such as a sugar pill or a saline injection, given either as a comparison in research or to harness the power of expectation. It looks and feels like real medicine but does nothing pharmacologically; any benefit it produces comes from the mind and the ritual of being treated, not from a chemical effect on a target.

In research a placebo is the essential control. By comparing an active drug against an identical-looking placebo, ideally with neither patient nor investigator knowing which is which, a trial can separate the drug's true effect from the improvement people show simply from being cared for, expecting to get better, or recovering naturally. Without this comparison, ordinary fluctuation in illness can masquerade as drug efficacy.

The placebo effect itself is real and measurable: expectation and conditioning can genuinely change symptoms like pain and nausea, partly through the body's own signaling. But it has limits and ethical boundaries. A placebo does not cure underlying disease, its effects are often modest and temporary, and deceiving a patient with one outside research raises serious ethical concerns. Its main scientific value is as the yardstick against which real drug effects are measured.

In a trial of a new painkiller, half the volunteers unknowingly receive identical-looking sugar pills; the drug must beat this placebo group to be judged effective.
