Pivotal Trial

The confirmatory clinical study, usually following feasibility work, designed and powered to provide the definitive evidence of safety and effectiveness that supports a marketing application such as a PMA. Compared with early feasibility studies, a pivotal trial is larger, uses prespecified primary endpoints and statistical success criteria, is often multi-site to support generalizability, and follows a locked protocol agreed in advance with the regulator.

Pivotal trials for invasive BCIs are genuinely hard: the eligible population is tiny and heterogeneous, blinding and sham controls are difficult or impossible, and every participant requires surgery, so classic large randomized designs may be infeasible. Investigators therefore use adaptive designs, objective performance criteria measured against historical or objective benchmarks, and Bayesian methods to draw rigorous inferences from small, hard-won cohorts.
