When you warm a part of the body, several things happen inside it at once — the same changes that explain why a heated muscle feels looser, less painful, and easier to stretch. Understanding these effects is what lets a clinician use heat deliberately rather than just for vague comfort.

There are three main effects. First, vasodilation: warmth widens small blood vessels, increasing blood flow, which can help carry away the chemical by-products of inflammation and bring nutrients in. Second, analgesia (pain relief): heat raises the threshold at which nerves report pain and helps quiet muscle that is in protective spasm, so the area simply hurts less. Third, increased tissue extensibility: warm collagen — the protein that makes up tendon, ligament, and scar — stretches more easily and is less likely to tear, which is why stretching is most effective in the few minutes right after heating, while the tissue is still warm.

These effects are also exactly why heat must be used with care. Increased blood flow is helpful for a stiff old injury but harmful for a fresh one, where it can worsen bleeding and swelling — so fresh injuries get cold, not heat. The warmth-driven extensibility window closes quickly once the tissue cools, so heat and stretch should be paired closely in time. And because heat blunts pain, it can mask a developing burn, which is dangerous over skin that cannot feel normally.

After a deep heating treatment of a contracted ankle, the therapist stretches it immediately while it is still warm, knowing the collagen's extra stretchiness will disappear within minutes as the tissue cools.

The very vasodilation that helps a chronic stiff joint is exactly what makes heat the wrong choice for a fresh injury, where increased blood flow worsens bleeding and swelling.
