Tmax

Tmax is the clock reading at the moment a drug hits its peak plasma concentration — in other words, how long after dosing the summit is reached. If you swallow a tablet at noon and the blood level peaks at 1 p.m., the Tmax is one hour. It captures the speed of the absorption-versus-elimination race rather than the height of the peak.

A short Tmax signals rapid absorption (fast onset), as with liquid or sublingual formulations; a long Tmax signals slow or sustained absorption, as with extended-release tablets or drugs taken with a fatty meal that delays gastric emptying. At Tmax the rate of absorption exactly equals the rate of elimination, which is why the curve is momentarily flat at its top.

Tmax is useful for matching a drug to a clinical need — a fast-onset analgesic for acute pain wants a short Tmax, while a once-daily controlled-release drug deliberately stretches it. Note that Tmax describes how fast, not how much; a slow formulation may share the same total exposure as a fast one while differing markedly in Tmax.

Sublingual glyceryl trinitrate for angina has a Tmax of just a few minutes, giving near-immediate relief, whereas an extended-release nitrate tablet peaks hours later.
