mechanism of action
Mechanism of action is the specific molecular story of how a drug produces its effect: which molecule it binds, what that binding does, and how that ripples outward into the change you observe. It is the answer to 'but how, exactly?' rather than just 'what does it do?'
A full mechanism connects the dots from molecule to outcome. For example, an opioid binds mu-opioid receptors on neurons, those receptors quiet the neuron's activity, the dampened signaling reduces transmission of pain messages, and the patient feels relief. Each link is part of the mechanism, from the initial binding to the downstream signaling cascade to the clinical effect.
Knowing the mechanism is powerful but not always necessary or complete. It lets pharmacologists predict drug interactions, side effects, and which patients will respond, and it guides the design of better drugs. Yet some genuinely useful drugs were used for years before their mechanism was understood, and for a few the mechanism remains only partly known. Mechanism explains a drug; it does not by itself prove the drug works, which requires clinical evidence.
